The Collaborating on Poverty Elimination Western New York, Inc. (COPE) is a 501 (c) (3) not-for-profit coalition of three partner organizations started in 2021. The vision was to integrate healthcare with community-based services to create a unique model for addressing the Social Determinants of Health and Health Equity. In 2023, the COPE Board of Directors re-imagined COPE’s strategy to focus on maximizing the social and systemic impact of the partner organizations.
